Begin Your Child’s Journey Toward Growth and Independence

Thank you for your interest in enrolling your child in the ABA/EIDBI therapy program at Wellness Therapy Center LLC. We’re honored to be considered as a partner in your family’s journey. Before submitting your application, please review the following eligibility and enrollment requirements carefully.

Enrollment Requirements

To ensure a successful start to services, families must meet the following conditions:

  1. Medical Diagnosis:
    Your child must have a medical diagnosis of Autism Spectrum Disorder (ASD) issued within the last 12 months.
  2. Active Insurance Coverage:
    Families must have active insurance that includes coverage for ABA, EIBI, or EIDBI therapies. Please refer to our FAQs or contact us for a list of accepted insurance providers.
  3. 6-Month Commitment:
    A minimum commitment of 6 months is required for enrollment to ensure continuity and effectiveness of therapy.
  4. Parental Involvement:
    Active caregiver participation is critical to your child’s success. This includes:
    • Parent meetings
    • Monthly family skills training
    • Quarterly treatment plan reviews
    • Parent Resource Group sessions
    • Ongoing communication with our team
  5. Transportation:
    Families are responsible for transporting their child to and from the center. Transportation support information is available upon request.
  6. Dietary Guidelines:
    While receiving services at our center, children are expected to follow a diet free from gluten, casein, and peanuts. This policy supports health and behavioral consistency.

Our Enrollment Process

Follow these five steps to begin services at Wellness Therapy Center LLC:

Step 1: Download and Submit Forms

  • Download the Enrollment Application Form from our website.
  • Complete all required fields and attach the following:
    • Recent medical diagnosis
    • Copy of your child’s insurance card(s)
    • Any supporting medical records

You may submit the completed form and documents via email or mail them to our physical address listed on the Contact page.

Step 2: Document Review

Once we receive your application, our administrative team will carefully review all submitted materials.

Step 3: Insurance Verification

We will contact your insurance provider to verify coverage and determine eligibility for ABA/EIDBI services.

Step 4: Intake Interview & Tour

Our Intake Coordinator will reach out to schedule:

  • A clinical intake interview with our therapy team
  • A guided tour of our therapy center and introduction to staff

Step 5: Waitlist Placement

After the interview, your child will be placed on our official waitlist, pending final clinical recommendations and scheduling availability.
